Health Canada Adds Pulmonary Aspiration Warning to All GLP-1 Receptor Agonist Product Monographs

Health Canada has completed a safety review linking GLP-1 receptor agonists — including semaglutide, tirzepatide, dulaglutide, liraglutide, and lixisenatide — to an increased risk of pulmonary aspiration during general anesthesia or deep sedation, and has updated the Canadian product monographs for the entire drug class. Community and hospital pharmacists are often the last clinician to see both the medication history and the upcoming procedure.

Health Canada has completed a safety review evaluating the potential increased risk of pulmonary aspiration in patients taking a GLP-1 receptor agonist (GLP-1 RA) who undergo a procedure requiring general anesthesia or deep sedation. The review covered the entire drug class — dulaglutide, liraglutide, lixisenatide, semaglutide, and tirzepatide-containing products, which include widely prescribed brands used for type 2 diabetes and chronic weight management. It was triggered by advisories from the Canadian Anesthesiologists' Society and the American Society of Anesthesiologists flagging the risk, and the findings were summarized in the April 2026 issue of Health Product InfoWatch. GLP-1 RAs slow gastric emptying as part of their therapeutic mechanism, and in patients undergoing sedation, retained stomach contents raise the risk of aspiration during airway management.

What changed

Health Canada worked with the manufacturers of all currently authorized GLP-1 RAs to update the Canadian product monograph safety information to reflect this risk, and is working with sponsors of newly approved products in the class to add the same language going forward. Unlike a recall or a Type I advisory, this is a labelling-level change: the drugs remain on the market and the benefit-risk balance for their approved indications is unchanged, but prescribers, anesthesiologists, and pharmacists now have a class-wide monograph warning to build perioperative counselling around.

Practice guidance for pharmacists

  • At initiation of any GLP-1 RA — for diabetes or weight management — tell the patient to inform every prescriber and their pharmacy before any future surgery, endoscopy, or procedure involving sedation.
  • When filling a pre-operative medication review or surgical clearance request, flag GLP-1 RA use explicitly rather than assuming the surgical team already knows, since these drugs are often started or refilled outside the surgical team's view.
  • Direct patients and prescribers to discuss individualized peri-procedural management with the anesthesia or endoscopy team rather than stopping the medication unilaterally — hold decisions depend on indication, dose, and timing of the last dose.
  • Update patient counselling materials for semaglutide, tirzepatide, dulaglutide, liraglutide, and lixisenatide products to reflect the new monograph language.
  • Watch for updated manufacturer product monographs and refresh any internally maintained patient education sheets accordingly.

GLP-1 RA prescribing has expanded rapidly across both diabetes and weight-management indications, and pharmacists are frequently the last point of contact before a patient's surgery or procedure. This monograph update gives pharmacists a concrete, citable basis for raising the aspiration-risk conversation proactively rather than waiting for the surgical team to ask.
